A 65-year-old woman went to the clinic after suffering from chronic knee pain for several months. She’d tried everything — including traditional Chinese medicine — but nothing helped. This X-ray was taken during her visit. What do you think the diagnosis is?Drop your answer in the comments

This is another engagement-style medical post. Without seeing the actual X-ray, it’s impossible to make a diagnosis responsibly.

A 65-year-old with chronic knee pain could have many possible causes, including:

  • Osteoarthritis (the most common cause in older adults)
  • Meniscus degeneration or tears
  • Ligament injuries
  • Crystal arthritis such as gout
  • Inflammatory arthritis
  • Bone spurs
  • Less commonly, infections or tumors

Why “guess the diagnosis” posts are problematic

An X-ray is only one piece of information. Doctors typically consider:

  • Symptoms
  • Physical examination
  • Medical history
  • X-rays and sometimes MRI or blood tests

Many different conditions can look similar on an image.

If I were evaluating the post

My first assumption would be osteoarthritis, simply because:

  • The patient is 65 years old
  • The pain has been chronic
  • It’s the most common explanation statistically

But that’s only a guess, not a diagnosis.

Bottom line

Without the X-ray image, nobody can accurately identify the condition. Even with the image, a proper diagnosis usually requires clinical context, not just a comment-section guess. If you can share the X-ray, I can explain what features doctors might look for and discuss the possible diagnoses.
